When Sanji was a child working on a ship, the vessel was wrecked in a storm. He and the pirate Zeff were stranded on a barren rock for eighty-five days. Zeff gave all the food to Sanji, telling him he had his own supply. When rescue came, Sanji discovered that Zeff's bag contained treasure, not food — the old pirate had starved himself so the boy could live, and had eaten his own leg to survive. That revelation — that a stranger chose to lose a limb rather than let a child go hungry — rewired Sanji's entire moral framework. He has never since been able to watch someone starve.

Sanji was born into the Vinsmoke family — a royal bloodline that genetically modified their children to be superhuman soldiers without emotions. Sanji's fighting style — Black Leg, modeled on French savate — uses only kicks because he refuses to risk injuring the hands that prepare food for his crew. The limitation is self-imposed and represents his priorities in physical form: cooking comes before combat. Feeding people comes before fighting people.
